1 This form is for Duke of Edinburgh’s Award notification purposes only and does not signify approval of the venture by the Authority.
2 This form should be submitted, to the Suffolk Scouts D of E Coordinator, 6 weeks prior to the commencement of any practice or qualifying venture that includes an overnight stay.
3 A NAN form must also be complete and sent to your DC two weeks prior to the event. Please also include a copy with this form.
4 For qualifying expeditions an appropriately qualified assessor must be named on the form along with the assessor number.
5 All practice and qualifying expeditions must be overseen by a D of E accredited supervisor.
6 Groups are reminded that pre-expedition checks must be carried out for all ventures.
7 For ventures into Wild Country/High Peak, the GREEN form should be used. Completed copies to be sent to the relevant wild country panel and a copy enclosed with this form.
8 For ventures abroad, please use the BLUE form. Please follow guidance on the form and return two copies to the area office no less than 12 weeks before the venture.
9 EXPEDITION VARIATION forms need to be completed 12 weeks before the planned expedition and sent to the Suffolk Scouts D of E Coordinator.
